We also record the date, the page you submitted from, and your IP address, to prevent abuse. This website is not intended for patient information, and we ask you not to submit any.
How we use it
We use it to respond to your inquiry, to provide the assessment or information you asked for, and to send you occasional information about our services. You can opt out of those emails at any time using the link in any email, or by contacting us.
Sharing
We do not sell your information. We share it only with service providers that help us operate, such as email delivery and customer-relationship tools, under contracts that restrict their use of it, or when the law requires.
Patient information
Protected health information we handle on behalf of client practices is governed by a Business Associate Agreement with each practice and by HIPAA, not by this website policy.
Mobile information
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text-messaging originator opt-in data and consent will not be shared with any third parties.
